• If you are a non-smoker with no medical conditions, complete and submit the online Client Information Form - this will enable us to source the best annuity for your needs. 
• If you are a smoker, or have any medical conditions which may affect your life expectancy, we could get you an even higher income.  If this applies to you, please print off the Client Information Form which includes a Medical Evidence Questionnaire.  . Once you have completed and signed your form, please return it to Bank of Scotland Annuity Service, Freepost SCE8362, Aylesbury HP21 7BR.  No stamp is required.
